Intramural Sports Forms

Intramural forms must be turned in and payment received for a team to be completely registered in an Intramural division for each sport. Payment should be made at the front desk area of the Student Recreation Center, where all paper work may be turned in at the same time. NO TEAM will be accepted without this process being completed.
